Dementia is commonly encountered in advanced stages of Parkinson disease, but evidence is accumulating that cognitive decline can manifest much earlier in the disease course. Aarsland and colleagues review current knowledge regarding cognitive impairment in patients with Parkinson disease, focusing on cerebrospinal fluid and imaging biomarkers as potential predictors of cognitive decline in this population. Cognitive enhancing medications have some effect in Parkinson’s dementia, but no convincing evidence that progression from mild cognitive impairment to dementia can be delayed or prevented is available, although cognitive training has shown promising results.
